A student requesting letters of recommendation from faculty, staff, guidance personnel, or administrators must complete the form below known as the Purple Form. The Purple Form will assist staff in completing the student's letters of recommendation for college admission and/or scholarship applications.

Completed forms should be turned in to the student's counselor.
